To enroll in your no cost One Medical membership program, we just need to know your preferences.
Your preferred One Medical office location (and PCP name, if you know). Find the office here.
Designate Hill Physicians as your Medical Group.
Take a short Initial Health Assessment (IHA) survey.
At no cost to you, your One Medical Membership can start on the first of the month after you complete these steps.
We will send you an email confirmation letting you know you can begin enjoying your One Medical membership, at no cost to you!
